IMPORTANT: Do not use this form if you can answer yes to any of the following questions – please dial 999 immediately:

• The crime is happening now.
• The offender is still there or nearby.
• Evidence has been left at the scene.
• Vulnerable, elderly or young victims are involved.
• The crime is of a violent or sexual nature.

If you wish to report a traffic accident do not use this form. Please attend your local police station or contact us for further advice on 01473 613500.

If you need immediate assistance always dial 999. You should dial 999 when life is threatened or people are injured, or if there is a crime in progress or an immediate police response is necessary.

Your completed form will be sent to our Crime Investigation Bureau (opening hours 7am to 10pm). When you have submitted the form you will receive a unique reference number. If you are the victim of the crime, a letter will be sent to you from Suffolk Constabulary, detailing the crime number.
